When you start noticing changes in a parent, it's hard to know what's normal, what's next and who to call. A Healthy Aging Roadmap gives your family a clear picture of where things stand today and a plan for the months and years ahead, so you're prepared instead of caught off guard.
Your Roadmap is built on a Comprehensive Geriatric Nursing Assessment. It goes deeper than a checklist. It looks at your loved one's health together with their goals and what quality of life means to them. And because I'm independent and don't work for the agency that decides which services your parent receives, my only focus is your loved one and your family.
Is this right for your family?

- Family members disagree about next steps and need a neutral, expert view.

Common questions
Is this a medical diagnosis?
No. This is a nursing assessment. I don't diagnose or prescribe, but with your permission I can share what I find with your loved one's family doctor.
How is this different from an AHS assessment?
Public assessments are mainly used to decide which services someone is eligible for. The Roadmap takes more time, centres on your loved one's goals and quality of life, and helps you plan for the whole road ahead.
Pairs well with: the Quality of Life Caring Guide and In-Home Safety Assessment.
