Most care plans are written around tasks and limitations: what someone needs help with and what they can no longer do. Those things matter. But they leave out the most important part, which is the person.

A Quality of Life Caring Guide is a written guide to your loved one as a whole person: their routines, preferences, relationships, history and joys. It gives everyone involved in their care, from family to home care workers to facility staff, what they need to support your loved one the way they would want.

Is this right for your family?

  • Your parent is starting home care, moving to supportive living or long-term care, or getting new caregivers.
  • You worry that new staff won't know "how Mom likes things."
  • Your loved one is living with dementia and can't always explain what they need or prefer.
  • Several family members share caregiving and you want everyone on the same page.

How it works

  1. Free 30-minute consultation. Tell me about your loved one and what you're hoping for.
  2. Visit (about 2 hours). I spend time with your loved one and family at home or in their care setting. We talk about their story, their days and what matters most. Your loved one is involved as much as they're able, at their own pace.
  3. I write the guide. I turn everything into a clear, warm, easy-to-read document.
  4. We review it together. You get a final copy to share with anyone who cares for your loved one.

What's in your Caring Guide

  • Life story highlights: family, work, faith or culture, proudest moments
  • Daily rhythms: mornings, meals, rest and bedtime routines
  • Personal care the way they like it
  • Favourite foods, music, activities and conversation topics
  • What brings comfort and joy, what can cause worry or distress, and what helps
  • Communication tips that work for them
  • The people and relationships that matter most
  • Their own goals and what a good day looks like to them

Common questions

Does this replace the care plan from AHS or the care home?

No. The Caring Guide works alongside the existing care plan. It adds the personal details that task-focused plans don't have room for.

Can my parent take part if they have dementia?

Yes. People living with dementia have a lot to tell us about themselves. I've spent over 20 years supporting people with dementia and adapt how I ask questions so your loved one's voice comes through.

Can the guide be updated later?

Yes. Needs and preferences change. I can update your guide as things shift.
